Form W-4 change requests need to be on an actual Form W-4 or a substitute Form W-4 per the IRS. A substitute Form W-4 must provide the tables, instructions, and worksheets contained in the Form W-4 in effect at that time. The substitute Form W-4 must contain language that's identical to the official Form W-4 and must meet current IRS rules for substitute forms.
W-4 changes requests in EWS do not provide a Form W-4 or a substitute Form W-4. Please change this.
