Requisition Submit Button - Hidden Submit To Field

In MIP Classic, when I hit the Submit button in Create/Approve Requisitions, there are hidden radio buttons and the Submit To field is not visible. I have to hit tab to see that the fields are there.

This happens when using my laptop. My resolution is 1920 x 1080. The "Change the Size of text, apps, and other items" display setting is set to 150% (Recommended), so that may be related.

I would attach a screenshot, but I received an error saying my organization won't allow it.
